Job summary

Spectrum Health in Michigan seeks a Supervisor of Community Health Programs to oversee CHWs, Social Navigation, and front desk ops within Healthier Communities South. You will drive chronic disease prevention, health equity, care coordination, and community engagement across care settings to improve access and outcomes.

The role requires a Bachelor's degree (Master's preferred) and 3 years of relevant program development experience; Spanish bilingual skills are a plus, with licensure

Qualifications

• Bachelor's degree required; advanced degree preferred.
• 3 years of recent relevant community program development required.
• Grant writing experience preferred.
• Bilingual Spanish preferred.
• RN or MSW license with state-specific eligibility may be preferred.

Responsibilities

• Supervises CHWs, Social Navigation services, and front desk operations to ensure program quality.
• Coordinates program planning, implementation, and evaluation with community partners.
• Act as liaison between clinical teams, community partners, and operational staff.
• Develops and monitors reporting, policies, and credentialing compliance.
• Identifies opportunities for philanthropic support to meet program objectives.
