Job summary

Roots Community Health Center seeks a Clinical Programs Specialist to drive care coordination, patient education, and proactive outreach for South Bay patients. You will collaborate with care teams, clinic staff, external providers, and community partners to manage a large patient panel and coordinate visits, lab monitoring, referrals, and outreach.

In this role you will maintain accurate patient records across EHR systems, support quality reporting, and participate in team meetings to address

Qualifications

• Strong interest in healthcare, social determinants of health, and health disparities.
• Experience with low-income communities of color.
• Excellent written and verbal communication skills.
• Able to work well in a team and with diverse backgrounds.
• Proficient in Microsoft Office and Google Workspace.
• Willingness to learn Electronic Health Records and multiple reporting systems.

Responsibilities

• Conduct external outreach to promote Roots programs and events.
• Coordinate care schedules, referrals, and testing with care teams and navigators.
• Maintain accurate daily service logs and program documentation across systems.
• Attend trainings and MAAs as required.
• Collaborate with leadership to identify and resolve barriers to care.
