Job summary

Description

Position Summary:

Representing Roots Community Health Center as a dedicated member of the Clinical Programs team, the Clinical Programs Specialist drives care coordination, patient education, and proactive outreach for South Bay patients. This role also supports cross-functional clinical initiatives that improve health outcomes for South Bay patients.

The Clinical Programs Specialist provides superior patient service, collaborating closely with care teams, clinic personnel, external specialty providers, and community partners. Managing a large patient panel, you will use advanced prioritization skills to proactively track and coordinate clinic visits, laboratory monitoring, specialty referrals, and community-based outreach. This position monitors, communicates, and documents all patient interactions across multiple electronic tracking systems, including Electronic Health Records. The Specialist role is also to ensure compliance with quality reporting requirements and deliver timely, impactful care.

Duties and Responsibilities:
Outreach, In-Reach & Community Engagement
  • Conduct external outreach to promote Roots programs, events, campaigns, and classes among community members and partner organizations.
  • Perform "in-reach" with existing patients to increase awareness, education, and active participation in clinical programs.
  • Support group visits, facilitate participant activities, and assist with incentive distribution.
  • Attend weekend outreach events and work on-site as needed.
Care Coordination & Patient Support
  • Collaborate with patient care providers and navigation team members to align on recommended visit schedules, share progress, and coordinate care.
  • Partner with Patient Care Coordination staff to verify insurance coverage, process referrals, and schedule appointments.
Documentation, Operations & Reporting
  • Maintain accurate, up-to-date daily service logs in program-specific patient tracking systems.
  • Work with administrative staff to ensure precise program documentation and reporting compliance.
  • Attend and participate in required role‑specific, organizational, and Medi‑Cal Administrative Activities (MAA) implementation training.
Strategic Collaboration & Issue Resolution
  • Participate in regular team meetings to discuss service coordination status, share insights, and address barriers like workload or hard‑to‑reach patients.
  • Provide program feedback to support strategic development and team improvements.
  • Identify and elevate complex issues to management for swift resolution.
  • Demonstrate flexibility in adapting to new processes, projects, and organizational priorities as needed.
Required Qualifications:
  • Strong interest in healthcare, social determinants of health, and health disparities.

Preferred Qualifications:
  • 1 year of experience performing clinical, coordination, and/or administrative functions.
