Job summary

Roots Community Health Center is seeking a Clinical Programs Specialist to drive care coordination, patient education, and proactive outreach for South Bay patients. You will support cross-functional clinical initiatives, collaborate with care teams and community partners, and manage a large patient panel with proactive tracking of visits, labs, referrals, and outreach.

The role emphasizes high-quality patient service, documentation, and compliance with reporting requirements, including MAAs

Qualifications

• Interest in healthcare, social determinants of health, and health disparities.
• Experience/comfort working with low-income communities of color.
• Excellent written and verbal communication skills.
• Strong teamwork and collaboration abilities.
• Ability to manage multiple phone lines, spreadsheets, and files.
• Ability to work effectively with patients from diverse backgrounds.
• Proficiency with Microsoft Office and G-Suite.
• Willingness to learn Electronic Health Records and multiple reporting systems.

Responsibilities

• Conduct outreach to promote Roots programs, events, campaigns, and classes.
• Perform in-reach with existing patients to increase engagement and participation.
• Support group visits and assist with participant activities and incentives.
• Attend weekend outreach events and work on-site as needed.
• Collaborate with care teams to align on visit schedules and referrals.
• Maintain accurate logs and ensure program documentation and reporting compliance.
