Job summary

BCFS Health and Human Services in Tyler, Texas seeks a Program Supervisor to directly supervise the Lead Case Manager, Case Managers, and Community Health Workers through QA and improvement activities such as home visits, weekly productivity meetings, and chart audits.

You will oversee intake and benefit assessments, goal setting, and care planning, coordinate with partner agencies, conduct health education sessions, and ensure accurate documentation in Salesforce while maintaining compliance

Qualifications

• Bachelor’s degree in a related field is required.
• Minimum of 2 years supervising staff.
• Valid Texas driver's license and clean driving record.
• Pass pre-employment drug screen and background checks.

Responsibilities

• Directly supervise Lead Case Manager, Case Managers, and CHWs.
• Oversee intake assessments, benefit assessments, goal setting, and case plans.
• Coordinate with partner agencies and participate in meetings.
• Conduct home visits, chart audits, and quality improvement activities.
• Ensure compliance with state/federal standards and document in Salesforce.
