Qualifications

• Preferred - 1 or more years of college-level coursework in a related field.
• Required – 2 years of job experience.

Responsibilities

• Outreach and recruitment of participants for the HEART program.
• Establish and maintain a trusting and professional working relationship with families by providing regular contact.
• Record all group and individualized services and activities in Salesforce (BCFS) databases.
• Maintain a list of applications’ status and update the Salesforce database.
• Provide safe, secure, and supervised transportation for the service population assigned to this position for transport.
• Provide referrals to participants based on needs identified during outreach efforts.
• Assist with coordinating and promoting HEART sponsored events and meetings, including classes and community events.
• Build collaborative relationships with the community, health care providers, community leaders, service agencies, policymakers, funding sources, and target audiences.
• Facilitate meetings and/or training for program participants.
• Communicate consistently with the program director regarding outreach concerns or issues.
• Maintain BCFS HHS professional standards of conduct and dress code.
