Job summary

The Salvation Army Ferguson Community Empowerment Center in Ferguson seeks a Pathway to Health Case Manager. You will recruit, enroll, and apply interventions for Pathway of Hope and Pathway to Health participants, with frequent local travel and community-based duties.

This full-time position offers 40 hours per week, a salary of approximately $46,000 annually, and involves coordinating with partner agencies to connect clients with needed resources while maintaining confidentiality.

Qualifications

• Bachelor’s degree in human services (BSW/MSW preferred) with 1 year related work experience.
• Outgoing personality with good communication skills; ability to work with diverse cultures.
• Strong organizational skills; knowledge of Salvation Army and local resources.

Responsibilities

• Recruit participants and build relationships with outside agencies for referrals.
• Conduct pre-screening and intake using specified assessment tools.
• Set goals with clients and conduct follow-up case management meetings.
• Record client information in SIMS & HMIS within 48 hours and prepare reports.
• Network with community agencies to maximize services and provide referrals.
• Attend seasonal events and provide emergency social services during disasters.
