Job summary

Golden Valley Health Centers seeks a Community Health Worker to support Atwater and Northview outreach efforts. You will educate communities, connect individuals with GVHC services, and represent GVHC at health fairs and events. The role is full-time and on-site, with flexible hours including weekends.

Strong interpersonal skills and bilingual abilities are required to engage diverse populations and coordinate with community partners to improve health access.

Qualifications

• High school diploma or equivalent.
• Reset
• 2+ years in community education, customer service or health care.
• Fluency in English and another language.

Responsibilities

• Inform patients or community members about health services using established curricula.
• Raising awareness of mental health needs through outreach.
• Attend and participate in community events as GVHC representative.
• Provide health screenings at events (e.g., blood pressure).
• Help connect patients with GVHC services and resources.
