Job summary

CommuniCare+OLE seeks a Community Health Worker to educate communities and connect patients and non-patients with health services. You will support outreach activities in Napa and surrounding areas, and work with our mobile unit to improve access to care.

The role emphasizes educating the public on available services, reducing health care gaps, and building trust with diverse populations. English/Spanish or Tagalog fluency is preferred, and a CA driver’s license is required.

Qualifications

• High school diploma or equivalent required.
• Associate or bachelor’s degree in Community Health, Human Services, or related field preferred.
• Minimum 1 year outreach/health education experience.
• Strong communication with patients, community, staff and management.
• Valid CA Driver’s License required.

Responsibilities

• Serve as liaison between community members and CommuniCare+OLE.
• Coordinate and execute calendar of outreach and health education activities.
• Educate hard-to-reach communities on services and resources.
• Travel to community locations for outreach activities.
• Maintain relationships with stakeholders and track outreach effectiveness.
