Job summary

Community Health and Wellness Partners in West Liberty, OH is seeking an Outreach Associate to serve as the first point of contact for patients. You will manage patient registration, collect demographic information, and support registration processes with a focus on accuracy and confidentiality.

You will assist with scheduling, billing coordination, and collaborate with team members while upholding CHWP’s mission and values.

Qualifications

• High School diploma or equivalent is required.
• English reading/writing; bilingual in Spanish helpful.
• BCLS Certified.
• Valid Ohio driver’s license.
• Knowledge of medical terminology preferred.
• Ability to manage multiple tasks with accuracy.
• Experience in customer service or public-facing roles.

Responsibilities

• Manage all patient registration and administrative processes, ensuring accurate and timely completion of registration records.
• Provide support for additional administrative and support activities within the registration/business office.
• Promote excellent customer service and ensure high levels of patient satisfaction.
• Ensure timely, accurate processing, entry, and filing of all records while maintaining confidentiality and HIPAA compliance.
• Develop and maintain positive relationships with team members, patients, vendors, contractors, and related resources.
• Serve CHWP’s Mission, Vision, and Values, representing the organization in a professional manner at all times.
• Volunteer to serve the community by offering five (5) hours annually to a community service program.
• Perform bed‑side registration, patient intake, and rooming.
• Prepare and maintain medical records and intake/patient information.
• Schedule visits, tests, and procedures; prepare requisitions for testing.
• Utilize computers, adding machines, fax machines, phones, and other office equipment to fulfill duties.
• Communicate verbally and/or in writing with staff, patients, and families.
• Answer patient inquiries about account information, insurance, co‑pays, etc., and collect co‑pays.
• Ensure all paperwork required for registration is prepared accurately and timely.
• Maintain accurate filing of all paperwork.
• Participate in meetings and training for professional growth.
• Support other CHWP activities and perform duties as assigned by the Clerical Outreach Coordinator.
• Support the Team‑care model when working with all CHWP staff.
• Perform other activities and duties as assigned by leadership or administration.
