Position Summary
Community Health and Wellness Partners (CHWP) is an integrated primary‑care community health center that utilizes a team‑based care model. The Outreach Associate is the first point of contact for all patients utilizing our facilities, manages the timely and accurate collection of important patient demographic information, and functions as a resource to patients entering the facility. They support and advocate for patients by assisting them with the registration process and conduct outreach efforts to implement financial assistance programs of CHWP.
Responsibilities
- Manage all patient registration and administrative processes, ensuring accurate and timely completion of registration records.
- Provide support for additional administrative and support activities within the registration/business office.
- Promote excellent customer service and ensure high levels of patient satisfaction.
- Ensure timely, accurate processing, entry, and filing of all records while maintaining confidentiality and HIPAA compliance.
- Develop and maintain positive relationships with team members, patients, vendors, contractors, and related resources.
- Serve CHWP’s Mission, Vision, and Values, representing the organization in a professional manner at all times.
- Volunteer to serve the community by offering five (5) hours annually to a community service program.
- Perform bed‑side registration, patient intake, and rooming.
- Prepare and maintain medical records and intake/patient information.
- Schedule visits, tests, and procedures; prepare requisitions for testing.
- Utilize computers, adding machines, fax machines, phones, and other office equipment to fulfill duties.
- Communicate verbally and/or in writing with staff, patients, and families.
- Answer patient inquiries about account information, insurance, co‑pays, etc., and collect co‑pays.
- Ensure all paperwork required for registration is prepared accurately and timely.
- Maintain accurate filing of all paperwork.
- Participate in meetings and training for professional growth.
- Support other CHWP activities and perform duties as assigned by the Clerical Outreach Coordinator.
- Support the Team‑care model when working with all CHWP staff.
- Perform other activities and duties as assigned by leadership or administration.

- Utilize a wide variety of office technology and tools to collect, enter, process, and file patient records and documentation.
Qualifications & Requirements
- High School diploma or equivalent.
- Ability to read, write, and speak English (bilingual in Spanish is helpful).
- Eligibility and outreach training as required.
- BCLS Certified.
- Valid Ohio driver’s license.
- Demonstrated skilled business‑office experience.
- Proven success in communication, customer service, or working with the general public, preferably in a medical care facility.
- Experience managing difficult customer situations; sound judgment and critical thinking/problem‑solving skills.
- General computer competence including basic Word and potential to be trained on specific software for patient information, billing, and communication.
- Knowledge of medical terminology is preferred.
- Ability to interact with a computer screen for up to six hours at a time.
- Manual dexterity for keyboard use; ability to remain stationary for up to four hours.
- Capacity to function in a sometimes stressful, multi‑tasking environment.
Work Environment & Hours
The position is a full‑time, non‑exempt role in a normal community health center environment (ambulatory primary care office). Work may require occasional weekend and/or evening hours. Hours are generally 8:00 a.m. to 7:00 p.m., Monday through Friday, with some Saturdays or after‑business hours work as needed. Assigned meetings and travel are part of the position.
Compensation & Benefits
Starting wages for the Outreach Associate are negotiable based on CHWP’s pay policies and factors such as education and experience. The position includes a ninety (90) calendar‑day probationary period; successful completion does not alter at‑will employment status.
