Connect Health + Wellness in Ridgeway, VA seeks a dedicated Outreach Coordinator to drive community outreach, patient registration, and promotion of services at mobile health unit events and school-based clinics.

The role supports new service lines, vaccination events, and partner coordination, ensuring accurate data in the EHR and guiding families to available care options. Strong organizational skills and data entry experience are essential.

Qualifications

• High School Diploma or equivalent required.
• Minimum of one year healthcare outreach, patient registration, community health, or related field preferred.
• Basic knowledge of health insurance, sliding fee scale programs, and patient eligibility preferred.
• Bilingual (English/Spanish) preferred.
• Prior experience in data entry required.
• A diverse set of skills and abilities including organization, prioritization, good grammar, telephone and interpersonal business manners and the ability to multitask.

Responsibilities

• Represent the health center at mobile health unit events, school-based activities, vaccination clinics, and community partner events.
• Promote CHW services, programs, and new service lines to patients, families, and community stakeholders.
• Assist with planning and execution of outreach events, including health fairs, school-based initiatives, and other outreach clinics.
• Register new and returning patients at mobile health unit and outreach events in accordance with FQHC and HRSA requirements.
• Obtain required patient consent, demographic information, insurance details, and sliding fee scale documentation.
• Ensure accurate and timely data entry into the electronic health record (EHR).
• Verify patient information and assist with identifying payer sources or self-pay eligibility.
• Attend all assigned mobile health unit events for the primary purpose of registration, outreach, and patient navigation.
• Serve as the on-site outreach and registration resource for patients and families.
• Coordinate with clinical and operational staff to support efficient patient flow during mobile events.
• Support school-based vaccination clinics and other immunization events in coordination with internal teams and community partners.
• Assist with consent collection, registration, and patient communication before, during, and after events.
• Provide families with education and information regarding services and follow-up care.
• Assist with outreach, patient education, and registration related to new service lines as they are launched.
• Support enrollment and patient engagement efforts for expanding programs.
• Support site-based initiatives aimed at increasing access, utilization, and continuity of care across all locations.
• Provide outreach and registration assistance for fixed-site clinic locations as needed.

About the Organization Connect Health + Wellness (https://connecthealthva.org/) is a Federally Qualified Health Center headquartered in Martinsville, Virginia, with locations currently open in Bassett, Martinsville, Patrick Springs and Ridgeway, Virginia. Connect Health + Wellness provides primary health, dental and integrated behavioral health services at our Federally Qualified Health Center sites and Dental sites by promoting health, reducing health risk factors and increasing access to medical and dental services, primarily for the uninsured and underserved in our service area. Additionally, we offer chronic disease self-management, medication assistance, and care coordination services to patients in our service area.

Outreach Coordinator

Connect Health + Wellness is seeking candidates for a full-time Outreach Coordinator position for our Ridgeway, Virginia location.

Purpose

Position Summary

The Outreach Coordinator is responsible for community outreach, patient registration, and promotion of services for Connect Health + Wellness. This role serves as a primary point of contact at mobile health unit events, school-based and community vaccination clinics, and other outreach activities. The Outreach Coordinator ensures accurate patient registration, supports new service line implementation, and strengthens relationships with community partners to expand access to care.

Essential Functions of the Position

Community Outreach & Engagement

• Represent the health center at mobile health unit events, school-based activities, vaccination clinics, and community partner events.
• Promote CHW services, programs, and new service lines to patients, families, and community stakeholders.

Pay Range

$15.00 to $17.00

Position Requirements
