Job summary

Community Health and Wellness Partners (CHWP) in Urbana, OH seeks an Outreach Associate to be the first contact for patients, manage registration, and support team-based care in a primary-care setting.

The role includes assisting with demographics, financial assistance, and accurate recordkeeping while upholding HIPAA and clinic policies. This is a full-time opportunity with variable hours to meet patient needs.

Qualifications

• High School diploma or equivalent.
• Must be able to speak, read, write, and understand English.
• Bilingual in Spanish helpful.
• Outreach/Eligibility training as required.
• BCLS certified.

Responsibilities

• Performs bed-side registration, patient intake, and rooming.
• Prepares and maintains medical records and intake information.
• Schedules visits, tests and procedures as directed.
• Communicates with staff, patients, and families.
• Ensures registration paperwork is accurate and timely.
• Maintains filing of all paperwork.
• Supports CHWP activities and training.
