Dartmouth Health is seeking a Community Health and Outreach Project Coordinator in New London, NH to support CHNA execution and CHIP implementation. You will partner with clinical teams and community organizations to identify needs and advance community health initiatives.

Responsabilités

• Serves as liaison between Dartmouth Health and New London Hospital to support community health and outreach.
• Coordinates community health programs and outreach activities; scheduling, logistics, stakeholder engagement.
• Maintains relationships with clinical teams and community partners to improve health outcomes.
• Assists with administration and distribution of funding to support social determinants of health programs.
• Supports tracking and reporting of community benefit activities for mandated reporting.
• Monitors CHNA and CHIP implementation on a three-year cycle with partners.
• Collects and communicates program outcomes and impact to stakeholders.
