North Country Hospital in Newport, VT, seeks a Community Health Team Supervisor / Chronic Care Coordinator II. This role splits about 50% supervision of the Community Health Team and 50% direct chronic care coordination for patients with complex needs, working across hospital services, primary care, and community partners.

The grant-funded position focuses on improving care transitions, reducing readmissions, and promoting positive patient outcomes through evidence-based care management

Formación

• Must possess and maintain a current, unrestricted RN or LPN license issued by the State of Vermont.
• Experience in a primary care setting.
• Experience working with local/state social services and health care agencies.
• Care management experience is a plus.

Responsabilidades

• Supervise and lead the Community Health Team (approx. 50%).
• Coordinate day-to-day ambulatory care management operations.
• Provide staff development, coaching, and quality improvement.
• Perform short-term chronic care coordination and complex case management (approx. 50%).
• Conduct patient assessments, care planning, and follow-up; coordinate resources and transitions.
