Yale NewHaven Health is seeking a Manager, Community Health to lead regional community health and health equity initiatives. You will assess needs, build partnerships with local organizations, and oversee CHNA/CHIP efforts while guiding system-wide health equity projects.

The role requires a Master’s in a relevant field and at least 5 years’ experience in community health, engagement, and program leadership.

Formación

• Master's degree in public health, business, public administration, health policy, or healthcare management preferred.
• 5+ years of experience developing, implementing, and overseeing outcomes of CHNAs, CHIPs and community-based programs.
• 5+ years of community engagement with partnerships, community members, community-based organizations and/or governmental agencies preferred.
• Strong ability to present and write for a broad range of constituents; collaborative in a matrixed environment.

Responsabilidades

• Lead Hospital region community health work and improve health equity with internal and external partners.
• Oversee CHNA/CHIP processes and track progress with metrics and reporting.
• Convene and manage collective impact partnerships and related committees.
• Provide data and education on community health to Hospital departments and leadership.
• Stay current on national/state trends and support dissemination of best practices.
• Collaborate with Marketing on annual communication plans and website updates.
• Manage Hospital community health staff and interns; coordinate special projects.
