Job summary

Community Healthcare Network in New York, NY seeks an Outreach and Engagement Specialist to support care management by connecting patients with community resources, following up on referrals, and completing intake screening.

You will educate patients about health conditions and available CHN services, coordinate with an interdisciplinary team, document encounters, and assist with arranging timely access to care.

Qualifications

• Associate Degree or equivalent with one year of community outreach or care coordination experience.
• High School Diploma or GED with three years of community outreach or care coordination experience.

Responsibilities

• Conduct new patient assessments consistent with the Scope of Services.
• Conduct outreach activities to newly assigned patients both in person and telephonically.
• Engage in outreach efforts to support patient retention in care coordination services and primary care, in person and by phone.
• Conduct community outreach visits to patients including at homes, shelters, hospitals and other community points of contact.
• Provide expedited visits to patients for urgent situations such as hospitalization.
• Inform patients of ancillary services and provide health education materials.
• Engage with Community Based Organizations to generate referrals to care coordination and other CHN services.
• Coordinate with the CHN interdisciplinary team to respond to internal referrals and attend care conferences.
• Notify Care Team of outcomes of assigned care coordination tasks.
• Assist the patient in selecting a Primary Care Provider or Mental Health Provider and arrange an initial visit.
• Provide patient with information on chronic conditions, mental illness, HIV prevention and primary care.
