Job summary

CommuniCare+OLE in Napa, CA is hiring a Community Health Worker to educate and outreach to communities, coordinate health education activities, and connect patients with services.

This full-time, nonexempt role requires a valid CA driver’s license, fieldwork ability, and bilingual skills (English/Spanish or Tagalog preferred). The position offers comprehensive benefits and a supportive, inclusive workplace.

Qualifications

• Education: high school diploma or equivalent; associate/bachelor preferred.
• 1 year of community outreach/health promotion experience.
• Ability to communicate with patients, community, staff and management.
• Valid CA Driver’s License required.
• Fluency in English/Spanish or Tagalog preferred.

Responsibilities

• Serve as liaison between community and CommuniCare+OLE.
• Coordinate outreach and health education activities in Fairfield and Suisun.
• Educate hard-to-reach communities on services and resources.
• Travel with mobile health team 2–3 days per week; some evenings.
• Support COVID-19 prevention and vaccination outreach.
• Share health promotion resources and information.
• Maintain relationships with stakeholders and community partners.
• Track outreach program effectiveness in Salesforce.
• Assist in quarterly grant program reporting.
• Deliver excellent customer service and responsive support.
• Collaborate effectively within a team.
