CommuniCare+OLE seeks a Community Health Worker to advance health access in Napa and surrounding areas. You will educate communities, support outreach events, and connect residents with our broad services from primary care to behavioral health.

The role is full-time in the Outreach department, requiring in-person collaboration, coordination of mobile unit activities, and strong communication with diverse populations.

Qualifications

• High school diploma or equivalent is required.
• Associate or bachelor’s in Community Health, Human Services, or related field preferred.
• Experience with CA health enrollment programs (preferred).
• Must communicate effectively with patients, community, staff and management.
• Minimum 1 year of outreach/health promotion experience; customer service.
• Fluency in English; Spanish or Tagalog preferred.

Responsibilities

• Serve as liaison between community members and CommuniCare+OLE.
• Coordinate and execute calendar of outreach and health education activities in the service area.
• Conduct public health education presentations regularly; participate in community events.
• Educate hard-to-reach populations on accessing services and resources.
• Travel to community locations to support mobile unit outreach 2–3 days per week.
• Maintain relationships with stakeholders and document outreach outcomes.
• Assist in grant reporting and program evaluation.
• Provide excellent customer service and teamwork.

Established in 2023, CommuniCare+OLE is the result of a union of two health centers with deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. JOB SUMMARY/OVERVIEW

TheCommunity Health Worker will beprovidingeducation,health promotion,outreach,and access to resources to patients and non-patients to increase health access in the cities of Fairfield and Suisun.Will also serve as the liaison between variouscommunity stakeholdersand CommuniCare+OLE to build trust and awarenessof the myriad of services we provide.The CHW will work in both the health center and community setting. This role will also be a part of CommuniCare+OLE ‘s Outreach team in supporting health education through CommuniCare+OLE ‘s mobile unit. Major functions of this role will include educating the public on the services offered at CommuniCare+OLE to reduce health care gaps in Fairfield and Suisun; increase healthcare access; help increase vaccination rates and COVID-19 outcomes for disproportionately impacted communities; and help build trust and awareness about CommuniCare+OLE.

MINIMUM POSITION REQUIREMENTS: EDUCATION, EXPERIENCE, SKILLS/TRAINING

• Education:
• High school diploma or its equivalent required
• Associate or bachelor's degree in Community Health, Human Services, or related field (Preferred)
• Experience/Lived Experience:
• Covered CA and Cal Fresh enrollment certification (preferred)
• 7Ability to communicate effectively with patients, community, staff, and management
• Must have a minimum of 1 year of experience working, conducting, and coordinating community outreach/health promotion and education
• Customer service experience
• Must have experience working with marginalized/hard-to-reach populations
• Knowledge of community health education and community events/health fairs
• Knowledge of the different community stakeholders (community partners, vulnerable and hard to reach communities, etc.
• Special Skills/Training:
• Strong verbal and written communication skills; Able to communicate fluently in English/Spanish or Tagalog (Preferred)
• Ability to communicate effectively with various stakeholders including patients, communitypartners,the community, andstaffand management
• Know and understand localhealthcommunityneeds,resources,andorganizations to help support and improve health outcomes

Essential Functions And Responsibilities

Serve as a liaison and bridge between community members and CommuniCare+OLE.

Helps coordinate and execute calendar of community outreach and health education activities in Fairfield and Suisun. Does public presentations on health education at least twice a month, and actively participates in community events three times per month. Ensures professional appearance of CommuniCare+OLE outreach activities.

Through CommuniCare+OLE’s mobile unit, in the health center setting, and through outreach at various community locations and events, the CHW will educate hard to reach vulnerable communities on accessing CommuniCare+OLE’s myriad of health-related service options, as well as connect them to community resources and services

Travels extensively to community locations and other outreach destinations with CCOLE’s mobile health team at least two to three days per week – evening, and weekend as needed

Assists in helping educate hard-to-reach populations on COVID-19 prevention and help build trust on the vaccine.

Sharing of information and resources for health promotion.

Maintains excellent relationships with various stakeholders including patients/non- patients and hard to reach vulnerable populations, as well as community partners.

Maintain, keep track, and report on outreach program effectiveness after each outreach activity in the Salesforce database.

Develop and help maintain quarterly grant program reports.

Provides excellent customer service to internal and external entities and treats everyone with dignity and respect. Seeks to assist community members with requests; solicits internal and external services; responds to requests for service and assistance; manages difficult or emotional patient situations; responds promptly to patient needs.

Works collaboratively and effectively within a team.

Performs other duties as assigned.
