Job summary

CommuniCare+OLE in Napa, CA is hiring a Community Health Worker to educate the public, promote health access, and coordinate outreach in Fairfield, Suisun, and surrounding areas. The role supports mobile health initiatives, vaccine education, and community partnerships.

The ideal candidate has strong communication skills, a willingness to travel, and a commitment to equitable health services for all residents. This is a full-time, nonexempt hourly position with benefits.

Qualifications

• High school diploma or equivalent required.
• Associate or bachelor's degree preferred in Community Health, Human Services, or related field.
• Minimum 1 year of experience in community outreach/health promotion and education.
• Strong communication with patients, community, staff and management.
• Knowledge of local health needs, resources, and organizations.

Responsibilities

• Serve as liaison between community members and CommuniCare+OLE.
• Coordinate calendar of outreach and health education activities in Fairfield and Suisun; present publicly at least twice monthly.
• Educate hard-to-reach populations on health options and connect to services.
• Travel to community locations with the mobile health team as needed (evenings/weekends).
• Assist in improving COVID-19 prevention and vaccine trust in communities.
• Maintain outreach program records and contribute to quarterly grant reports.
