Job summary

COPE Health Solutions ECM Program in Merced, CA is seeking a Community Health Worker to help patients navigate services, provide social support, and advocate for community health needs. You will collaborate with care teams and document interactions in the EMR.

The role involves outreach, scheduling, addressing social determinants of health, and follow-up visits; Spanish language skills preferred; valid CA driver's license required; travel up to 80%.

Qualifications

• Valid California Driver’s License.
• High school graduate or equivalent; Associate's Degree preferred.
• Community Health Worker training program preferred.
• Written and oral fluency in English and Spanish preferred.
• Experience in a multi-cultural setting.
• EMR experience preferred.

Responsibilities

• Educate members about ECM services and assist enrollment.
• Support individuals and families navigating the health care system.
• Establish trusting relationships with patients and families.
• Provide follow-up, motivational interviewing, and goal setting.
• Bridge conversations and remove barriers to health and social services.
• Conduct face-to-face outreach for appointment scheduling and care gap closure.
• Meet members in clinic, facility, or at home to assess social determinants of health.
• Collaborate with the care team to create individualized care plans.
• Assist with accessing health-related services and resources.
• Document interactions in the EMR within 24 hours.
• Manage assigned caseload and maintain HIPAA compliance.
