You will supervise the HEART Community Wellness Advocate and Outreach Team, oversee hiring and professional development, and collaborate with Program Leadership to optimize workflows and data collection.

The role emphasizes coordinating with nursing staff, managing schedules and resources, and implementing standard procedures to sustain and scale community health efforts.

Qualifications

• Bachelor’s degree or equivalent; master’s preferred in healthcare admin or related field.
• Minimum 3 years of progressive management or leadership experience.
• Experience coordinating clinical and community health operations is desirable.

Responsibilities

• Oversee day-to-day operations of the HEART Community Wellness Advocate and Outreach Team.
• Coordinate with clinical staff to ensure seamless transitions between medical care and community resources.
• Manage program logistics, schedules, supplies, and staffing for daily operations.
• Implement operational protocols and supervise frontline staff for practicality and effectiveness.
• Monitor program activity metrics and report to leadership to meet enrollment and outreach targets.
