You’ll partner with primary care teams to manage recently discharged patients, address urgent needs to prevent ER visits, and oversee chronic disease management and preventive care in collaboration with center-based providers.

Qualifikationen

• NP/PA with unrestricted license.
• DEA license required.
• Experience in care transitions, high-risk or medically complex patients preferred.

Aufgaben

• Provide community-based medical and care-coordination services for recent hospital discharges.
• Conduct patient assessments and reviews of medications and plans of care.
• Coordinate with home care agencies and the primary care team.
• Educate patients and families on treatment plans and health maintenance.
• Document in the Electronic Medical Record and ensure privacy and security.
