Job Juncture in the Greater Sacramento area is seeking a full-time Nurse Practitioner or Physician Assistant to deliver comprehensive primary care in a community health setting. You will work with physicians and a multidisciplinary team to manage acute and chronic conditions, provide preventive care, and ensure documentation meets regulatory standards.

Qualifikationen

• Current California RN license
• Nurse Practitioner license (CA)
• Current DEA certification
• Current BLS certification
• 1–2 years clinical experience, preferably in community health/underserved populations
• Commitment to serving low-income, homeless, or underserved communities
• EPIC EMR experience
• In-basket management competency
• Bilingual English and Russian or Farsi/ Pashto/Spanish preferred
• Malpractice tail coverage provided at no cost

Aufgaben

• Perform comprehensive patient evaluations including history and physicals
• Diagnose and manage acute and chronic conditions
• Order and interpret labs, imaging, and diagnostics
• Prescribe medications and therapies within scope
• Develop and implement treatment plans with Medical Director
• Monitor progress and adjust care plans
• Initiate emergency procedures when needed
• Provide preventive care, education, and counseling
• Maintain accurate documentation per regulatory standards
• Collaborate with physicians and care teams for coordinated care
