Job summary

Qualifications

• High School Diploma, Transcripts, or Equivalent.
• Associate’s Degree in healthcare-related studies preferred.
• Two years healthcare setting experience preferred.

Responsibilities

• Help patients identify social determinants of health that affect their health and develop health and social management plans.
• Assist patients with SMART goals and navigate social services.
• Outreach to assist patients in overcoming barriers to care and social services.
• Build relationships with community services and establish a presence in new communities.
• Arrange transportation and medical appointments for patients.
• Complete outreach and follow-up calls to patients.
