Qualifikationen

• Identify social determinants of health affecting patients and help develop health and social management plans.
• Assist patients in setting SMART goals and addressing barriers to care.
• Conduct outreach and assist patients in accessing medical care and social services.
• Build relationships with community services and establish a presence in new communities.
• Arrange transportation and medical appointments for patients and conduct follow-up calls.
