• Conduct outreach in community settings to build trust and expand access to care.
• Work with diverse populations and support patients across the continuum of care.
• Identify and recruit patients for services.
• Provide counseling, health education, and harm reduction support.
• Assist with care navigation and addressing barriers to insurance access and affordability.
• Support medication access, including applications and enrollment assistance.
• Link patients to HIV care or prevention services and provide follow-up.
• Perform clinical tasks including specimen collection (venipuncture, testing, urine collection) and HIV/STI counseling/testing.
• Accurately document services and contribute to program goals.
