La Clinica De Familia is looking for a Community Health Worker in Las Cruces, NM. This position under the Women's Clinic Administrator involves patient support through telephonic and in-person help, acting as a liaison to enhance access to healthcare services.

The right candidate will engage with vulnerable populations, demonstrate strong communication, and be community-connected. An equivalent high school degree and experience in clinical settings are required, alongside the ability to obtain certification early in employment.

Qualifications

• 1+ years of experience in a physician's office or clinical setting.
• Obtain Community Health Worker Certification within the first month of employment if not currently certified.
• Proficient computer skills.

Responsibilities

• Support patient navigation through telephonic and in-person support.
• Liaison between individuals and healthcare services.
• Address social determinants of health by facilitating access to care.

Skills

Experience with Medicare/Medicaid

Effective communication skills
