Job summary

Carle Health is seeking a Community Health Worker to connect underserved communities with health and social services. The CHW will partner with community agencies, provide education, assist with navigating systems, and coordinate care under supervision.

Qualifications

• BLS certification within 30 days (AHA).
• Driver's license from Illinois (ILSOS).
• Minimum 2 years of community outreach experience.

Responsibilities

• Assess assigned clients at scheduled intervals and screen Social Determinants of Health.
• Advocate for clients and update care plans to facilitate adherence and positive outcomes.
• Educate clients on how to obtain care and self-manage their conditions.
• Refer and connect clients to needed health services and resources.
• Document all client interactions and assessments; maintain a client caseload.
• Attend interdisciplinary care team discussions and engage in community outreach to raise program visibility.
• Work with the CHW Supervisor to identify, develop and implement outreach strategies for high-risk populations.
