Job summary

Your Health is seeking a dedicated Community Health Worker to service patients in Johns Creek, GA. The CHW will visit homes and ALFs, deliver medications and DME, and coordinate visits with providers to ensure timely care.

Reliable transportation and daily travel are required on a full-time schedule with 12-hour shifts (7:00 AM–7:00 PM). Join a leading physician group delivering quality care across Georgia and South Carolina.

Qualifications

• Certified Medical Assistant, Paramedic, or Licensed Nurse preferred with current licensure.
• Phlebotomy experience preferred; willing to train if not experienced.
• Experience in public health or home health is advantageous.
• Proficiency with remote patient monitoring, telemedicine, and electronic medical records.

Responsibilities

• Coordinate and facilitate home and ALF/ILF visits to enhance patient care.
• Deliver medications and DME, ensuring safe administration and patient education.
• Arrange and support telehealth visits between patients and care teams to ensure timely care.
• Assist with RPM setup and ongoing patient education and troubleshooting.
• Travel daily to Johns Creek, GA service area and other assignments as needed.
