Job summary

Your Health Organization in Georgia is seeking a dedicated Community Health Worker to serve patients in Lawrenceville and surrounding areas. The CHW will be a vital link between patients, healthcare services, and community resources, performing home visits and assisting with medications and DME to ensure timely access to care.

This full-time role requires reliable transportation, a clean driving record, and the ability to work M-F, 8:00 AM–5:00 PM, with travel between offices and facilities.

Qualifications

• Certified Medical Assistant, Paramedic, or RN preferred.
• Phlebotomy experience preferred; willing to train if needed.
• Experience in public health or home health preferred.
• Proficient with technology tools for RPM, telemedicine, and EMR.
• Excellent organizational and communication skills.

Responsibilities

• Perform home and ALF visits to support patient care.
• Deliver medications and DME to patients' homes.
• Coordinate telehealth visits with providers and pharmacists.
• Reconcile medications and ensure accurate documentation.
• Travel between offices and care facilities as required.
