Job summary

Your Health Organization in Georgia seeks a dedicated Community Health Worker (CHW) to serve patients in Johns Creek, Duluth, and surrounding areas. The role includes home visits, DME delivery, and coordinating with providers to ensure access to care.

Daily travel and a 8-hour M-F schedule are required, with competitive compensation and benefits. Qualified candidates maintain clinical skills, can coordinate telehealth, and support patients in navigating healthcare resources.

Qualifications

• Certified Medical Assistant, Paramedic, or Licensed Nurse preferred.
• Phlebotomy experience preferred; willing to train if not experienced.
• Public health or home health experience preferred.
• Experience with DME delivery or logistics preferred.
• Proficient with technology tools for telemedicine and EMR.

Responsibilities

• Work with care team to ensure coordinated patient care and adherence to care plan.
• Communicate effectively via phone, email, Teams, and in person.
• Coordinate visits with PTs, OTs, and Providers in facilities and homes.
• Deliver medications, review instructions, and reconcile medications.
• Coordinate telehealth visits with pharmacists and specialists.
• Set up RPM equipment training and provide ongoing support.
• Deliver DME to homes/ALFs and train patients and caregivers.
• Perform venipuncture and collect specimens when needed.
• Travel daily between offices, facilities, and patient homes.
