Job summary

Your Health Organization in Athens, GA is seeking a dedicated Community Health Worker (CHW) to serve patients in the Athens area. The CHW will visit patients at homes and ALFs, help coordinate visits with providers, and assist with delivering medications and durable medical equipment (DME) to ensure timely access to care.

The role requires reliable transportation and daily travel, is full-time, and follows a Monday–Friday 8:00 AM–5:00 PM schedule.

Qualifications

• Certifications or licenses must be in good standing (as required).
• Experience with home health or public health preferred.
• Must have reliable transportation and ability to travel daily.

Responsibilities

• Visit patients at their homes and ALFs to coordinate visits with providers.
• Deliver medications and DME, review instructions, and reconcile medications.
• Coordinate telehealth visits between patients and pharmacists/physicians.
• Set up RPM equipment training and provide ongoing support and troubleshooting.
• Perform venipuncture and collect blood specimens as needed.
• Work with care team to ensure coordinated patient care and compliance with care plans.
