Job summary

Your Health Organization is seeking a dedicated Community Health Worker to support patients in the Athens and Lake Hartwell area. The CHW will visit homes and ALFs, coordinate care with providers, deliver medications and DME, and help patients access services.

Travel is daily; you must have reliable transportation. This full-time position offers salary-based compensation, 12-hour shifts from 7:00 AM to 7:00 PM, and opportunities to contribute to high-quality community healthcare.

Qualifications

Certified Medical Assistant, Paramedic, or Licensed Nurse preferred and in good standing.
Phlebotomy experience preferred; willing to train if not experienced.
Experience in public health or home health preferred.
Experience with DME delivery or logistics preferred.
Proficient with technology tools for remote monitoring, telemedicine, and EMR.
Strong organizational and time management skills; able to multitask.
Excellent reading, writing, and verbal communication.
Basic computer knowledge; able to work independently and travel as needed.
Licensed driver with insured vehicle and clean driving record.

Responsibilities

Work with care team to ensure coordinated patient care per care plan.
Coordinate visits with PT/OT and Providers in ALFs and patients' homes.
Deliver medications to patients' homes, review instructions, and reconcile meds.
Coordinate telehealth visits between patients and pharmacists, specialists, and PCPs.
Set up training for Remote Patient Monitoring equipment and provide ongoing support.
Deliver Durable Medical Equipment to homes and ALFs; provide training on use.
Perform routine venipuncture and handle specimens.
Collect blood specimens during home or facility visits.
