The role involves intake support, coordinating medical and behavioral health appointments (including telehealth), medication monitoring, and documenting health information in the electronic health record,

Competenze

• High School diploma or equivalent required.
• One year of caregiving/human services experience preferred.
• Must apply for Agency Affiliated Counselor registration within 30 days of hire and maintain active status within 90 days.
• First Aid/CPR certification within 90 days of hire.
• Strong written and verbal communication skills.
• Basic computer skills and ability to learn new software.
• Demonstrates professional, empathetic, client-centered approach in recovery-focused settings.

Mansioni

• Coordinate and monitor client physical and behavioral health services across residential facilities.
• Assist with intake processes, appointments (including telehealth), and medication monitoring.
• Document health information accurately in the electronic health record and communicate with clinical staff.
• Support care coordination under nursing direction where available and under Health & Resource Coordination leadership where not.
• Engage clients, reinforce treatment plans, and support self-care and health education.
• Maintain a safe, clean, and compliant health services environment.
