Job summary

Albuquerque Health Care for the Homeless is seeking a Community Health Worker to support clients on campus and in outreach, helping navigate care systems, obtain benefits, and access housing.

Working within a client-centered, trauma-informed, and harm-reduction framework, you will provide short-term care coordination, health education, and connect individuals to long-term care networks, collaborating with medical and social services teams.

Qualifications

• Experience in social services or healthcare settings.
• Ability to obtain NM CHW certification within 90 days.
• Experience with co-occurring medical conditions.
• Valid NM driver’s license with clean driving record.

Responsibilities

• Explain care plans and available services to patients.
• Document encounters and plans in EHR within 72 hours.
• Submit monthly reports and maintain notes and releases.
• Refer unengaged patients to Harm Reduction, Dental, Behavioral Health, and Case Management.
• Assist with identifying socio-economic issues and develop health/social plans.
• Connect patients to community resources and housing assistance.
• Assist with benefits applications including Medicaid.
• Collaborate with medical and social services teams.

Skills

Social services experience

NM CHW Certification within 90 days

High school diploma or equivalent

Education

Associate degree in Human Services or relevant field
