Job summary

The Wright Center for Community Health and Graduate Medical Education seeks a Community Health Worker to partner with clinicians, social services, and patients to coordinate care and connect resources. You will support health education, home visits, and outreach events to improve patient well-being and access to care.

Responsibilities include documenting encounters in EMR, assisting with care plan development, and coordinating with providers to ensure timely referrals and follow-up.

Qualifications

• High school diploma or GED required; 3+ years in related duties preferred.
• BLS certification required.
• Experience with care coordination and community outreach preferred.
• Bilingual candidates are highly desired.
• Knowledge of patient-centered medical home and social determinants of health.

Responsibilities

• Assist patients in homes, community, or health center settings.
• Communicate program purposes and expected impact to patients.
• Help identify socio-economic issues affecting health and develop care plans.
• Document encounters in EMR and submit monthly reports.
• Educate patients on proper ED use and self-management of chronic conditions.
• Assist with referrals and coordination of services with providers and social services.
