Job summary

HealthLinc in Elkhart, Indiana is seeking a Community Health Worker to support the healthcare team by coaching patients, families, and caregivers to improve self-management and health outcomes through motivational interviewing.

You will connect patients with community resources, schedule follow-ups, referrals, transportation, and help coordinate care with the medical team, while maintaining culturally competent service.

Qualifications

• High school diploma or equivalent required.
• Formal CHW training program completed or in progress.
• Chronic Care Professional (CCP) certificate preferred.
• Spanish interpretation cert optional but highly valued.

Responsibilities

• Establish trusting relationships with patients and families while providing general support and encouragement.
• Provide ongoing follow up, basic motivational interviewing and goal setting with patients/families.
• Serve as liaison between clinical staff and patient, by following up with appointments, referrals, and needed resources.
• Help patients set personal goals and attend appointments and provide referrals to community services as appropriate.
• Assist patients in connecting with transportation resources and provide appointment reminders.
• Collaborate with the care team to ensure comprehensive, coordinated care.
• Promote culturally and linguistically competent services; understand community culture and resources.
• Conduct outreach and implement community health programs; collect data to identify health needs.
