Job summary

AdvantageCare Physicians (ACPNY) is a growing primary and specialty care network in the New York area seeking a Community Health Worker to bridge gaps between care systems and patients. You will conduct health and social needs screenings, build rapport with vulnerable populations, and coordinate referrals to community resources to improve access and outcomes.

Qualifications

Responsibilities

• Conduct and manage social determinants of health screenings and upload results.
• Engage patients to identify health and social needs and assess barriers to care.
• Coordinate care and connect patients to community and clinical resources.
• Provide health education and promote self-management in a culturally appropriate manner.
• Maintain documentation and track patient progress in electronic systems.
• Collaborate with interdisciplinary care teams to address patient needs.
• Performs other job-related duties as required.
