Qualifications

• High School Graduate or Equivalent.
• Minimum of 3 years’ experience in home health, hospice, managed care, or social service agency.
• Active driver’s license and ability to drive in Ohio.
• Preferred: Community Health Worker Certification.

Responsibilities

• Assist care coordinators with accessing resources, coordinating referrals, and providing community education.
• Create connections between vulnerable populations and healthcare providers.
• Help patients navigate healthcare and social service systems.
• Reduce social isolation among patients.
• Determine eligibility and enroll individuals in health insurance plans.
• Provide culturally appropriate health education on chronic disease prevention, physical activity, and nutrition.
• Advocate for underserved individuals to receive services addressing social determinants of health.
