Acacia Network is seeking a Health Home Navigator to provide intensive care management in New York, advocating for clients and coordinating services with Care Managers. You will conduct field visits, prepare documentation, and assist in eligibility processes to promote health and reduce ER visits.

The role emphasizes outreach, enrollment support, and strong communication with clients, families, and providers, with a focus on wellness and empowerment.

Formación

• Associate’s/ Bachelor’s Degree strongly preferred.
• High School Diploma or GED; OR
• CASAC; OR
• Community Health Worker.

Responsabilidades

• Complete a minimum of five clients contact per day.
• Participate in weekly supervision to review referrals for the week, enrollment, remove barriers to meet enrollment quota.
• Advocate aggressively for clients to obtain full range of needed service and ensure coordination of these services.
• Assist Health Home Care Managers with gathering Health Home enrollment consents, RHIO consents, eligibility, and appropriateness assessments.
• Assist in the integration of Health Home with then Acacia Network based on eligibility and appropriateness screenings.
• Conduct home visits, hospital, and clinic visits to provide thorough support to enrolled and potential members.
