Oak Street Health is hiring a Community Health Worker to bridge patients with community resources and medical systems. You will promote health literacy, advocate for patients, and collaborate with the care team to support individualized care plans.

The role requires strong communication, flexibility, and knowledge of the local community. A valid driver’s license and access to reliable transportation are needed to travel to community sites.

Qualifications

• Minimum 1 year in healthcare, community-based, case management, or social services.
• Strong oral and written communication skills.
• Ability to manage multiple priorities while maintaining a positive attitude.
• Dedication to serving the community and building relationships.
• Proficient computer skills (Windows, GSuite, Microsoft).
• Access to reliable transportation and ability to travel across the community.
• US work authorization.

Responsabilités

• Establish and maintain relationships with patients and community partners to coordinate needs.
• Manage patient referrals with the care team and action plans.
• Facilitate communication among patients, families, providers and community organizations.
• Build inventory of local community resources for patients' needs.
• Connect patients to housing, transportation, food, and daily living resources.
