Job summary

Oak Street Health seeks a Community Health Worker to bridge patients with healthcare systems and community resources. You will advocate for patients, conduct outreach, and support care planning with the care team to promote health literacy and improve access to services. Role involves coordinating with Medical Social Workers, assisting with benefit applications, and visiting patients in centers, homes, or community settings.

Qualifications

• Minimum of 1 year of experience in healthcare, community-based, case management, or social service environment.
• Strong oral and written communication skills and ability to multitask.
• Willingness to travel throughout the community and maintain reliable transportation.
• Access to US work authorization and proficiency with standard software tools.

Responsibilities

• Establish and maintain relationships with patients, community organizations, and care teams.
• Coordinate referrals and action plans with the Medical Social Worker.
• Facilitate communication among patients, families, providers, and community resources.
• Educate patients and connect them to housing, transportation, and other services.
• Document interactions in the electronic medical record with HIPAA compliance.
• Spend about half of time outside the center in patient-centered locations.
