Job summary

Mobridge Regional Hospital & Clinics is seeking a Community Health Worker to act as a bridge between health care services, social services, and the community. You will advocate for patients and support them in reaching their health goals within clinic and community settings.

The role focuses on health navigation, resource coordination, and education, requiring a high school diploma, CPR certification, and SD CHW training. A valid driver's license is required.

Qualifications

• Minimum of a high school diploma or equivalent.
• Prefer a minimum of two years of applicable experience.
• Will complete a paid online approved CHW training program in South Dakota and CPR certification.

Responsibilities

• Health system navigation and resource coordination within clinic or community settings.
• Act as a patient advocate and help patients reach their health goals.
• Assist patients in finding providers and scheduling appointments for services.
• Arrange transportation to medical appointments.
• Connect patients with social services, counseling and support groups.
• Provide health information and education to patients.
• Document patient information and progress in the Electronic Medical Records system.
• Contribute to health booths and community outreach presentations.
• Work independently and as part of a team; assist during Public Health emergencies.
