Job summary

Mobridge Regional Hospital & Clinics seeks a self-motivated Community Health Worker to serve as a non-medical member of the patient-care team and advocate for patients’ best interests. You will coordinate care, connect patients with services, and help them reach health goals in clinic and community settings.

You will document progress in the EMR, educate patients on resources, and work with families, medical providers, and social services. Requires high school diploma and valid driver’s license.

Qualifications

• Minimum of a high school diploma or equivalent.
• Prefer a minimum of two years of applicable experience.
• Willing to complete CHW training and CPR certification.
• Must have a valid driver's license and clean driving record.
• Ability to communicate professionally with medical staff and diverse communities.

Responsibilities

• Health system navigation and resource coordination within clinic/community.
• Advocate for patients and help schedule appointments and connect with services.
• Arrange transportation for patients to medical appointments as needed.
• Link patients to social services, counseling, and community resources.
• Assist with insurance navigation and provide health information and education.
• Document patient information and progress in the EMR system.
• Possibly participate in health booths or presentations; work independently or in a team.
• Support in the event of a Public Health emergency.

Mobridge Regional Hospital & Clinics is seeking a self-motivated team player to join our rural healthcare organization.Successful applicants must have a passion for helping others. We are looking for someone with a strong desire to use their own education and life experiences to serve as an intermediary between health care, social service organizations and the community. Their role will be to act as a non-medical member of the patientcare team and advocate on behalf of their patients’ best interests while helping them reach their health goals.

This individual will work closely with patients, families (support network), MRH&C medical providers, primary care team, and social servicesagencies to facilitatecare coordination. As well as a connection to resources thatsupport patients to improve their health, self-sufficiency, and general well-being. The Community Health Worker will be working with a wide variety of patientsfrom all backgrounds in both clinical and community-based settings.

Primary Job Responsibilities:

• Health system navigation and resource coordination, such as:
• Working as a patient advocate primarily within the clinic or community setting.
• Helping patients find providers and assisting patientsto make appointments for various services. Includingconnecting patients with necessary specialty provider appointments as recommended.
• Transporting and/or arranging transportation for patients to get to and/or from medical appointments.
• Possibly attendingappointments with patients for medical services.
• Linking patients/clients to other relevant community resources, such as social services, counseling and support groups.
• Assistingwith insurance navigation
• Provide appropriate health information and education to patients, such as:
• Providing health education to patients as ordered by their medical provider.
• Complete social determinants of health screenings and educating patients on resources (transportation, food, support services, etc.).
• Ensuring patients’ own support network, including friends and family understand health information and education provided.
• Other skills and abilities:
• Ability to connect with and provide ongoing support.
• Will document patient information and progress in the Electronic Medical Records system.
• Will conduct presentations or assist with health booths.
• Ability to work independently without supervision and as part of a team.
• Staff may be asked to assist in the event of a Public Health emergency.
• Minimum of a high school diploma or equivalent.
• Prefer a minimum of two years ofapplicable experience.
• Will complete a paid online approved CHW training program in South Dakota and complete other mandatory training, including CPR certification.
• Must have a valid driver’s license and a clean driving record.
• Experience with navigating local medical and social support systems is desired.
• Demonstrated organizational and time management skills
• Applicant must be able tocommunicate professionally and effectively both verbally and in writing with medical staff, including providers, and other professionals, Social Service agency staff as well as people from all cultural andsocio-economic backgrounds.
• Ability to pass a thorough background investigation.

This is a full-time 40-hour per week position, which could include some evening work.
