Job summary

Sierra Vista Child & Family Services in Merced is seeking a Community Health Navigator to promote health education and preventive care in underserved communities.

The CHN will increase awareness of services for Medi-Cal members, provide outreach, and refer resources to prevent disease and reduce emergency visits. Join a nonprofit team dedicated to health equity, collaboration with multidisciplinary staff, and opportunities to impact families.

Qualifications

  • High school diploma or GED required.
  • Proficient in Microsoft Office (Word, Excel, PowerPoint).
  • Knowledge of community resources.
  • Valid California driver’s license required for staff who have regular driving duties.

Responsibilities

  • Utilize prevention strategies to promote stigma and discrimination reduction, suicide prevention, mental health awareness and well-being.
  • Provide access, advocacy, resources and linkage to clients and their families.
  • Interact productively with a multi-disciplinary team and other community organizations.
  • Maintain record and tracking systems that meet standards of the agency, The Joint Commission and contract.
  • Must implement program goals and objectives to meet and maintain contractual requirements and productivity.
  • Effectively use de-escalation skills and techniques for crisis prevention.
  • Properly administer the C-SSRS to help prevent suicide.

A Community Health Navigator (CHN) focuses on promoting community-based health education and prevention in underserved communities. The Community Health Navigator aims to improve health equity for Medi-Cal members through health education and resource referrals by providing outreach witha focus on preventive health services to prevent disease, disability and other health conditions or their progression; to prolong life; and promote physical and mental health and well-being. CHN’s increase awareness of services, promote preventative care and reduce emergency visits.

Qualifications
  • HighSchooldiploma or GED required.
  • Proficient in Microsoft suite (Word, Excel, PowerPoint).
  • Knowledgeofcommunity resources.
  • A valid California driver’s license is required of staff who have regular driving duties and the driving record must meet the requirements of their position and be approved by the agency’s insurance company.
