Health Care Management, Inc. seeks a Community Health Worker / Care Manager to join our Enhanced Care Management and CHOW program.
You will conduct face-to-face and telehealth visits, connect clients with health and social services, and support care planning with a multidisciplinary team. You must have a high school diploma, a valid California driver’s license with a clean DMV record, a personal vehicle, and CPR within 30 days.

You will be part of theEnhanced Care Management andCommunityHealthOutreach Work program (CHOW). TheCommunityHealthWorker/Care Manageris a trusted member of thecommunitywho serves as a link betweenhealth, social services and thecommunityto facilitate access to services and improve the quality and cultural competence of service delivery. You will work with the program team comprised ofcommunityhealthworkers, care managers, clerical support, and program managers/supervisors to deliver services.
You can work in the community for face-to-face and telehealth/phone contact visits with clients. You have excellent communication and basic knowledge of the geographic community, knowledge of available support and services within the community, and an ability to research services that may be available to your caseload.
This is a full-time position, 40 hours per week, from Monday through Friday from 8am to 5pm with an hour for lunch. No weekend work.
This is not a remote role and requires you to work in our Sacramentooffice.
You will need your own car and be able to travel to clients' homes.You will be traveling in Sacramento and surrounding areas, such as Midtown Sacramento, West Sacramento, Sutter, Solano, San Joaquin, Yolo and Placer counties.
PPE(Personal Protective Equipment)is provided to all employees.
Transporting clients with personal vehicle is prohibited.
