Job summary

Rebekah Children's Services is seeking a Temporary Medical Care Navigator to support our outpatient clinics in Campbell, CA. The role assists in meeting clients' health needs, coordinates medication referrals, and supports psychiatrists in delivering timely care.

Strong bilingual skills and crisis-intervention abilities are preferred to engage families effectively. The position requires CNA/MA/LVN credentials, at least 2 years of direct service with children and families, and knowledge of

POSITION SUMMARY

Under the direction of the Chief Clinical Officer or Director of Community Services and following established procedures, assists in meeting clients' health needs and receives and coordinates referrals for medication services. The Medical Care Navigator’s primary responsibility is the support of the Psychiatrist in accomplishing their tasks in a timely manner and supporting client care through organizational and nursing skills. Job performance contributes to the achievement of the goals of RCS’ mental health program’s provision of successful services and outcomes.

TYPICAL RESPONSIBILITIES AND DUTIES:

• Receives and reviews referrals for psychiatry.
• Engages families and referring clinicians within 1 business day of receiving the referrals.
• Completes intake paperwork and is responsible for ensuring clients' timely access to Psychiatric services by following the guidelines set forth by the county and state regarding timelines for processing referrals, contacting clients, and setting up initial intakes. Responsible for tracking, monitoring, and reporting data that measures progress towards these timelines.
• Uses a variety of culturally competent engagement strategies to engage families in services.
• Reviews each client’s insurance coverages and confirms scheduled appointments with Psychiatrists. Tracks all indicated medical records as well as necessary consents for medications prescribed by Psychiatrists.
• Coordinates with Social Worker and Probation officers to receive court-ordered consents for medication.
• Communicates with pharmacies to ensure prescriptions are delivered correctly.
• Supports in the training of staff in CPR and other medical-related processes.
• Supports in the collection and sharing of information and data required for treatment and outcomes using a variety of assessment, outcome, and satisfaction tools.
• Supports with greeting clients and community members at the agency front desk.
• Takes and documents vitals of each client seen by Psychiatrist.
• Obtains and documents clients' height, weight, blood pressure, pulse rate, waist measurement, and BMI to effectively track his/her growth and development, monitor for adverse effects, and to meet regulatory and/or licensing requirements when applicable. Documents and reports any fluctuations outside the normal range to the Psychiatrist.
• Provide sound customer service and direction regarding client questions and feedback.
• Supports with translation when needed during intake, assessments, etc.
• Completes data entry and client screenings including using both internal and external databases.
• Supports children and families navigating in Medi-cal system, including eligibility requirements and processes.
• Generates reports including but not limited to referral logs, access to services and utilization summaries.
• Supports in the coordination of department and agency events.
• Demonstrates a high level of competence in working with children and families to assist them in attaining permanency, safety, well-being, and their self-determined quality of life.
• Ability to engage clients in a manner that is culturally competent and dynamic and abides by time-sensitive deadlines.
• Provides quality, culturally proficient, trauma-informed, and relevant ancillary services as part of a treatment team.
• Possesses knowledge of community-based resources and the ability to link and refer families to resources in the community based on their assessed needs.
• Documents necessary information as required within the Program in a timely manner and consistently meets or exceeds agency productivity standards, including Intake Forms.
• Maintains professional standards regarding client rights.
• Demonstrates an understanding of confidentiality policies, mandated child abuse reporting, and duty to warn procedures. Utilizes clinical supervision in any case where these issues are in question.
• Demonstrates the ability to think creatively and intervene/responds in crisis situations.
• Utilizes supervision, consultation, and training in providing ongoing high-quality services to children and their families effectively and efficiently.
• Demonstrates the ability to work independently. Actively uses supervision and consultation from his/her supervisorand other team members.
• Provides assistance to other Care Navigators by filling in as needed.
• Aids in the administration of TB examinations for the agency.
• Every employee has the opportunity and responsibility to participate in one or more activities each year that support the performance quality improvement (PQI) of RCS programs and services. Activities might involve membership on a PQI committee or workgroup, service as a peer reviewer, data collection for PQI and evaluation purposes, completion of PQI questionnaires and surveys or participation in focus groups, or other activities as identified by PQI committees or work groups.

MINIMUM QUALIFICATIONS:

EDUCATION, TRAINING AND EXPERIENCE:

• Certified Nurse’s Aide (CNA), Medical Assistant (MA), or Licensed Vocational Nurse (LVN) preferred.
• 2 years of direct service with children and families within the mental health field, preferred.
• Must meet the Title 9, Title 19, and Title 22 Medi-Cal Regulations
• Experience working in a children’s mental health setting emphasizing a family systems perspective desirable.
• CPR Certified
• Basic skills in crisis intervention, suicide assessment, conflict resolution, and interpersonal communication.
• Bilingual/ Bicultural (Spanish/English) required.

LICENSE(S):

• California driver’s license and good DMV report (and ability to be insured by RCS’ insurance carrier).

KNOWLEDGE AND SKILLS:

• Demonstrated ability to provide quality, culturally competent and relevant educational and preventive services to at-risk children and families.
• Strong sensitivity to the diverse cultural needs of children and families served.
• Proficient oral and written communication skills.
• Proficient organizational and time management skills.
• Ability to work independently exercising good judgment, make decisions & problem solve.

• Proficiency in Microsoft Office applications.
• Must have reliable transportation, valid driver’s license and minimum state required automobile insurance.
• Must be available evenings and/or weekends as required to fulfill job responsibilities.

SPECIAL REQUIREMENTS:

• Must be able to meet and receive a criminal records clearance as required by Title XXII licensing regulations.

PHYSICAL AND MENTAL REQUIREMENTS:

The following are required in day-to-day performance of the duties of this position:

• Sitting, walking, standing, grasping (simple), and visual/auditory acuity are constantly required (over 2/3 of the workday).
• Bending, squatting, kneeling, twisting, lifting, reaching, and pushing/pulling are required occasionally (under 1/3 of the workday).
• Climbing and crawling are generally not required.
• Driving/Traveling to client locations such as school, home and/or community setting.
