Job summary

Sierra Vista Child & Family Services in Modesto, CA seeks a Case Manager I/II for our Drop-in Family Resource Center to deliver early intervention and prevention services for youth and families.

You will meet youth at home or school, coordinate with community resources, help implement treatment plans, and support positive behavior changes. This role requires a flexible schedule, including some evenings and Saturdays.

Qualifications

  • High school diploma or GED required.
  • Bachelor's degree in Counseling, Social Work or related field required. Equivalent experience accepted.
  • Knowledge of community resources preferred.
  • Bilingual required.
  • A valid California driver’s license is required for staff with driving duties; driving record must meet requirements.

Responsibilities

  • Operate a vehicle for client visits, meetings, trainings and other appointments.
  • Ensure driving requirements are met, including license validity, vehicle registration and insurance as required.
  • Communicate ideas effectively, both orally and in writing.
  • Instruct groups and individuals as part of program delivery.
  • Maintain physical, mental, and emotional health adequate to perform duties.
  • Possess knowledge of mental health programs and community resources.
  • Adhere to applicable legal and ethical standards and protect confidentiality.
  • Work harmoniously with staff and diverse communities.

Position Summary

This position provides early intervention and prevention services focused on diverting vulnerable, high-risk youth from the juvenile justice system into community-based, developmental services and supports so that youth break the cycle of delinquent and criminal behavior and activity. The Case Manager ensures that youth coming to the family resource center receive caring and professional case management services. The Case Manager assists youth and their families with implementation of recommended treatment plans, so they can re-focus their attention on positive and productive behavior and choices. The Case Manager will meet with the youth in their home and at their school with a flexible schedule.

  • A valid California driver’s license is required of staff who have regular driving duties, and the driving record must meet the requirements of their position and be approved by the agency’s insurance company.
Essential Job Functions
  • Operate an agency or personal vehicle for work-related activities which may include, client visits, meetings, trainings, or other agency-related appointments
  • Ensure compliance with driving requirements, including maintaining a valid driver’s license, current vehicle registration, and proof of insurance as required by agency policy.

Responsibilities
  • Provide access, advocacy, resources and linkage to clients.
  • Attend community events, provide outreach services to include identifying areas that need to be strengthened, in addition to setting and reaching targeted goals. May need to work evenings and weekends as needed.
  • Administer program measurement tools and surveys to track and report program data and outcomes.
  • Interact productively within a multi-disciplinary team and other community organizations.
  • Maintain record and tracking systems that meet standards of the agency, The Joint Commission and contract.
  • Responsible for preventing and controlling infection.
  • Responsible for maintaining a culture of quality and safety.
  • Effectively use de-escalation skills and techniques for crisis prevention
  • Properly administer the C-SSRS to help prevent suicide.
  • Interaction is appropriate and sensitive to client’s culture. Interaction is appropriate with client’s developmental level.
Other
  • Attend all required meetings and training.
  • Report any suspected child or dependent adult/elder abuse or neglect immediately to direct supervisor or utilize the chain of command if supervisor is unavailable.
  • Report any client imminent danger to self or to others or gravely disabled immediately to direct supervisor or utilize the chain of command if supervisor is unavailable.
  • Other duties as assigned.

Base Pay: $21.00-$28.38/hour
