Job summary

ChildNet Youth and Family Services seeks a Bilingual Lead Care Manager for the Enhanced Care Management (ECM) Program in Palm Desert, CA. The LCM leads care management, coordinates services, and serves on the interdisciplinary Care Team to promote wellness, resilience, and access to care.

A flexible, compassionate professional with Spanish and English fluency will supervise casework and advocate for clients.

  • Bachelor's degree in a social/human services-related field or equivalent experience.
  • Bilingual Spanish required; strong English proficiency.

Responsibilities

  • Assess client needs across physical health, mental health, SUD, housing, and social supports.
  • Develop and implement Individual Care Plans/Health Action Plans.
  • Provide services where clients are located (office, telehealth, or field-based).
  • Coordinate with health care providers and community services to ensure timely access to care.
  • Attend team meetings and complete documentation within agreed timeframes.

Job description

ChildNet Youth and Family Services is currently seeking a Bilingual Lead Care Manager for our Enhanced Care Management (ECM) Program located in Palm Desert, CA.

Under the direction of the Enhanced Care Management Supervisor, the Lead Care Manager (LCM) provides direct service, as well as coordinates care management, and functions as a part of a "Care Team" for the Enhanced Care Management Program (ECM). The LCM oversees specific cases, coordinates health care benefits, provides education, and facilitates client access to care in a timely and cost-effective manner. The LCM collaborates and communicates with clients, caregivers/family support persons, and other providers to promote wellness, recovery, independence, resilience, and empowerment, while ensuring access to appropriate services and maximizing client benefit. The LCM also serves as an advocate for clients, an active client of the interdisciplinary team, and a liaison with other programs and external health and social service providers in the community.

The LCM responsibilities include, but are not limited to, the following:
  • Assess client needs in the areas of physical health, mental health, SUD, oral health, trauma-informed care, social supports, housing, vocational/employment, wellness, and referral and linkage to community-based services and supports.
  • Oversee the development and implementation of the Individual Care Plan/Health Action Plan
  • Offer services where the client lives, seeks care, or finds most easily accessible, including office-based, telehealth, or field-based services.
  • Connect clients to other social services and supports that are needed (e.g., community support group).
  • Coordinate/advocate on behalf of client with health care professionals (e.g., PCP), as permitted.
  • Utilize evidence-based practices, such as Motivational Interviewing, Harm Reduction Techniques, and Trauma- Informed Care principles.
  • Work collaboratively with hospital staff regarding Transitional Care Planning
  • Conduct outreach and engagement activities to facilitate linkage to the ECM program. Outreach and Engagement consists of phone calls, mailed information, and field visits.
  • Accompany clients to office visits, as needed and appropriate.
  • Evaluate progress and update goals.
  • Provide mental health promotion.
  • Arrange transportation
  • Complete all documentation within the timeframes established by the individual action plans
  • Attend weekly staff/team meetings and supervision.
  • Attend training as assigned (e.g., ACEs Certification).
  • Perform any other duties as assigned.
The Lead Care Manager (LCM) must possess:
  • Bachelor's degree in a social/human services-related field of study; OR
  • High school diploma/GED and at least 3 years of relevant work experience.
  • Good knowledge of the resources of this community.
  • Ability to complete the training program and ongoing educational requirements as assigned.
  • Ability to work flexible hours, including occasional night/weekend work.
  • Able to communicate in English clearly and effectively both verbally and in writing.
  • Bilingual proficiency in speaking, reading, writing, and understanding Spanish is required.
  • Prior experience as a Community Health Worker, Peer Support Specialist, Medical Assistant, or similar role is a plus.

All offers of employment with ChildNet Youth and Family Services, Inc. are contingent upon a post-offer, pre-hire medical clearance with an occupational physical, tuberculosis screening, and a negative drug test. A criminal background clearance which consists of fingerprinting through the Dept. of Justice/FBI/Child Abuse Index Search is required. An essential function of this position does require driving. Underwriting requirements of this agency include keeping a valid California driver's license, valid auto liability insurance on file, and maintaining a good driving record while employed.
