ChildNet Youth and Family Services seeks a Bilingual Lead Care Manager for the Enhanced Care Management (ECM) Program in Palm Desert, CA. The LCM leads care management, coordinates services, and serves on the interdisciplinary Care Team to promote wellness, resilience, and access to care.
A flexible, compassionate professional with Spanish and English fluency will supervise casework and advocate for clients.
Under the direction of the Enhanced Care Management Supervisor, the Lead Care Manager (LCM) provides direct service, as well as coordinates care management, and functions as a part of a "Care Team" for the Enhanced Care Management Program (ECM). The LCM oversees specific cases, coordinates health care benefits, provides education, and facilitates client access to care in a timely and cost-effective manner. The LCM collaborates and communicates with clients, caregivers/family support persons, and other providers to promote wellness, recovery, independence, resilience, and empowerment, while ensuring access to appropriate services and maximizing client benefit. The LCM also serves as an advocate for clients, an active client of the interdisciplinary team, and a liaison with other programs and external health and social service providers in the community.

A criminal background clearance which consists of fingerprinting through the Dept. of Justice/FBI/Child Abuse Index Search is required. An essential function of this position does require driving. Underwriting requirements of this agency include keeping a valid California driver's license, valid auto liability insurance on file, and maintaining a good driving record while employed.
Hourly rate: $24.00
